Conservation Surgery for Laryngeal Cancer


Abstract
Conservation surgery of the larynx refers to surgical removal of malignant tumors involving specific areas of the larynx with preservation of sphincteric, respiratory and phonatory function.
In 1865, Dr. H.B. Sands had successfully removed a small glottic cancer via thyrotomy and had thereby his place in the history of conservation surgery of the larynx. Ogura et al. listed the following reasons why conservation surgery can be done in treatment of carcinoma involving the true vocal cords or supraglottis: 1) slow growth with tendency to remain localized to the primary site, 2) a paucity of lymphatics in the true vocal cord, 3) distinct lymphatic pathways above and below, 4) independent resectability of regional node metastasis.
Small malignancies may be excised endoscopically with the CO2 LASER, by performing a block resection, using the laser beam to incise through normal tissue at the margins of the block.
The purpose of this report is to make sure historical background of conservation surgery for laryngeal carcinomas and to reveal some clinical demonstrations.
